D2 Survivors runs only on your own Diablo II files, out of respect for Blizzard's work. The desktop app plays from your own install: nothing is downloaded from Blizzard, and the app holds no Blizzard file. On first launch it finds your Diablo II and Lord of Destruction and extracts their art and sound once, in about a minute and a half. From then on it plays offline.

Open the .dmg and drag D2 Survivors into Applications. It keeps itself up to date.

The AppImage is the app itself: allow it to run as a program (its Properties, or chmod +x) and open it. It keeps itself up to date. On a Steam Deck, in desktop mode, add it to Steam as a non-Steam game to start it from game mode too. Diablo II runs there in Wine or Proton: if the app can't find it, point it at its prefix (a non-Steam game's is under ~/.local/share/Steam/steamapps/compatdata). Ctrl+H shows hidden folders.

On the website, SAVES saves your progress as a file or a code, with or without game files (on a phone too). In the app, SAVES imports it.
